Identify: John Paul II and Benedict XVI Exonumia

Please help me identify this token. I have looked all over the Internet but not found a match. I have found some vaguely similar Vatican City tokens, but nothing quite right.

It is 23mm in diameter. Unfortunately, I do not know its composition, diameter, or anything about its origins.

I presume it was to commemorate the transition from Pope John Paul II to Pope Benedict XVI and issued by Vatican City (maybe). But oddly is says ‘An. XXVI’ which I believe is 2004 not 2005 when John Paul II passed away - assuming this refers to his regnal years.

Any information about it would be greatly appreciated. Thank you in advance!

XXVI is 26 and that how many years he was Pope. Second longest, Pope Pius XI was the longest at 31 years. Wikipedia

It's great to see the catalogue becoming more and more comprehensive.  For me, it's also quite cool to see your own tag at the bottom of a picture on a brand new page.  I hope you get the same enjoyment as I do, both in finding a new unlisted token or coin and then having it verified.
