According to the “Record of Congregations” (page 11, here), Knox's Galt Presbyterian Church, Cambridge had its origins in 1883 (this should read 1832) with the formation of St. Andrew's Presbyterian Church, in connection with the Church of Scotland. In 1844, the majority withdrew from St. Andrew's to join the Presbyterian Church of Canada. The minority entered a lawsuit to retain the church building and so, the majority took the name Knox's and constructed their own church building."
The Cambridge, Ontario website says, "The City of Cambridge was incorporated in 1973, when the three municipalities of Galt, Preston and Hespeler along with parts of Waterloo and North Dumfries Townships were amalgamated into a single legal entity under a new name."
This (communion) token is larger and heavier than the Leroux type listed here.
